New Patients
Once your appointment is scheduled, you will receive two emails:
Patient Intake Form
Health History Form
Both emails will have the subject line: “Dental Forms”
Please check your junk/spam folder if you do not see them within 24 hours of booking.
If you have not received these emails, please contact us as soon as possible at 289-483-1143. These completed forms are required to finalize your registration with our office.
Returning Patients
A new Health History Form is required:
Every 12 months, or
Any time there is a change in your health.
If it’s been over a year since your last visit, you’ll receive the form automatically by email. If it hasn’t been a full year, but your health has changed (e.g., medications, diagnoses, allergies), please inform our Administrative Team at 289-483-1143 so we can ensure you receive an email to update the information.
